BCS-SCF Application Forms

The following forms are provided below for easy access if you are interested in submitting an application for certification. Both the BCS-SCF Application Continuing Education Form and the BCS-SCF Application Direct Clinical Activity Form may be utilized as an efficient way to document both educational and clinical hours as you progress towards becoming eligible to submit an application. (See Standards for Certification for more information). Once an application is submitted and accepted, access to all forms necessary for Portfolio Submission is granted to the candidate.
